NOTICE IS HEREBY GIVEN TO John Doe, said name being fictitious, father, as well as Jane Doe, said name being fictitious, mother, whereabouts unknown:
Pursuant to 22 M.R.S. §4001 et seq., the Maine Department of Health & Human Services (hereafter, "DHHS") has petitioned the Court for a Child Protection Order and/or Termination of Parental Rights Order, concerning the child: Safe Haven Baby Girl n/k/a Grace A. S., d/o/b: July 1, 2016, born at Bangor, Maine.
The mother is Jane Doe, and the father is John Doe.
DHHS has met the requirements of M.R. Civ. P. Rule 4(g)(1)(A)-(C). Hearing on the pending Petition(s) will be held at Penobscot Judicial Center, 78 Exchange Street, Bangor, Maine, 04401 on December 16, 2016, at 2:00 p.m. Failure to appear at this hearing may result in the issuance of a child protection order, the termination of your parental rights, and/or any other order permissible under 22 M.R.S. §4001 et seq.
